Health Center Intern Business Card Order Form
An LCCW standard business card is established for the use of all students currently enrolled in the College. All cards include the Life West address & phone.
Please note that the cost for 1,000 cards with shipping is a flat rate of $40. This will be deducted by the Life West Business Office from your financial aid.
Please fill out the form below to order your Life West branded business cards. Once your request is approved, your request will be sent to the College design partner and printed.
